Samsung N8 LCD (LA46N81B/DX) Service menu Settings

My Samsung LCD has been acting weird of late and the service menu appeared to be corrupt. I was hoping somebody could please tell me the default service menu settings for this TV, more specifically the Panel Vendor, Model Option, Model Type settings.
